It is advised that if this is your first clinic examination, please bring any pertinent medical record information with you to better assist your physician.

We keep your records in strict confidence and will not disclose their contents without your expressed written permission. Before we will release information in your medical record to any third party, such as your insurance company, you must sign an authorization for such a release.
